I plan to participate in Lakewood High School’s 2012 Graduation ceremony at Colorado University Events Center on Saturday, May 26, 2012, at 10:00 a.m. I agree to meet the following requirements:

A. I will attend graduation rehearsal at 9:00 a.m., Friday, May 25, 2012, in the school

gymnasium.

B. I will assume all financial obligations for cap, gown, and graduation fee.

    C.              I will wear ONLY the authorized graduation cap, gown, and tassel. In addition,

                      graduates may wear up to two honor cords and one academic medal (if earned). Students will

                      not add any other accessories, decorations, or carry any placards, posters or any other

                      paraphernalia that might distract from the ceremony.

    D.              My attire under my gown will be suitable and will reflect the importance and dignity of the

                      occasion (i.e. black slacks and black dress shoes for men and dress shoes for ladies).

E.              I will not engage in any act of rowdiness, noisy behavior, or misconduct. I will conduct myself

                      properly before, during, and immediately following the ceremony so the graduation ceremony

                      will be meaningful to everyone in attendance.

   F.             I will abide by the Board of Education Policy regarding alcohol and narcotics before, during and

                       immediately following the ceremony.

G.     I will report to CU Events Center no later than 9:00 a.m. on Saturday, May 26^th.

H.              I will fulfill 22 credits by May 26^th.

I.                The Graduation Fee of $65.00 WILL NOT be refunded after May 11^th

**I understand that if I deliberately violate any of the above conditions, I will forfeit my right to participate in the ceremony. I further understand that in the event of my violation of any of the above, I will be required to leave the graduation ceremony.

Graduation begins promptly at 10:00 a.m. Please mark you calendar.

***Caps and gowns will not be issued to students who have fines***
